Today I want to share a summary of two questionnaires. First Post traumatic stress disorder (P.T.S.D). Questionnaires are filled out by those evaluating you. There is a Diagnostic Summary. Current Diagnose. They will list the disorders with a code. Ask if there are other diagnoses that need added. Comments in some areas. If they answer no they need to provide a reason. If yes, what attributes to them. Do they have a TBI Traumatic Brain Injury?
Occupation and social impairment. Again, there will be a yes or no question relating to this. Also needing an explanation. Clinical findings. They will then answer some questions by check off the ones they feel you have that. Then your symptoms will be checked off as well. Behavioral observations will be filled out by the examiner. They will list any other symptoms that attribute to the PTSD. They are asked about your competency. If the answer relating to that is no. They must fill out the reason why. Last, any remarks.
Second questionnaire is for mental disorders other than PTSD and eating disorders. Diagnosis. Differentiation of symptoms. Occupational and social impairment. Clinical findings. History. Check of symptoms you have. Behavioral observations. Other symptoms. Competency and remarks.
